1-Nursing History o Collect data about erectile dysfunction in a male who has diabetes and may be indicated in the review of the cardiovascular, neurologic and genitourinary system.Nursing history data indicating the need for a physical examination include the following: 1- Suspicion of infertility, pregnancy or STDs 2- Reports of discharge, presence of lump or change in color, size and shape of a genital organ.o Disfiguring conditions such as burns, skin conditions, birth marks, scars, and ostomies Specific medication therapy that decreases sexual drive or causes erectile or ejaculatory dysfunction.3-Identifying clients at risk o Physical, psychosocial, or sexual abuse Altered body structure or function due to trauma, pregnancy, recent child birth, anatomic abnormalities of the genitals, or disease.
