In later gestation, precursors for maternal estrogen and androgen production such as pregnenolone and progesterone come from the fetal adrenal glands.3 Therefore, biomonitoring maternal hormone concentrations during pregnancy provides valuable information regarding the dynamic fetal production.4 Urinary steroidal profiling as an alternative, noninvasive way has shown promising application in clinical diagnosis.5, 6 To facilitate the understanding of steroid hormones and their biological and pathological functions, it is essential to develop a sensitive method that allows comprehensive determination of urinary hormones.

Many analytical methods have been proposed for the detection of steroid hormones in biological specimens. Although immunoassays have routinely been used to measure steroid hormones in clinical applications because of the method's simplicity and sensitivity,7 limitations of the method including poor specificity and the limited dynamic range. Due to the low volatilities of hormones, the general application of gas chromatography/mass spectrometry (GC/MS)-based analysis was hampered by the time-consuming derivatization step.8 Liquid chromatography coupled with tandem mass spectrometry (LC/MS/MS) has increasingly been adopted as it provides superior analytical sensitivity, specificity, accuracy and wide dynamic range.6, 9-11 In addition, LC/MS/MS enables simultaneous determination of various steroids at low concentration in a single analytical run.9, 12

Steroid hormones are present in urine almost exclusively as conjugated metabolites such as glucuronides, sulfates, diglucuronides, disulfates and sulfoglucuronides.13 Conjugated forms (sulfates or glucuronides) of steroid hormones are considered as hormone precursors because they can easily cross cellular membranes and transform into biological fluids.13, 14 Therefore, the objective of the present study was to develop a LC/MS/MS method that could simultaneously determine the hormones (total, free plus conjugated forms) in urine samples. The method showed promise of application in clinical diagnosis and metabolomics areas of the steroid hormones.
