Nursing assessment (diagnosis and prescription):
Orem self-care model has suggested that by seeking information from
clients, the nurse will be able to make decisions regarding the planning and
to implement actions of care. The nurse must assess fives main areas:

1. Those demands being made on the elder for self-care.


2. The elder's ability to meet these demands.


3. If a self-care deficit does exist, the nurse establishes the reasons for it.


4. Assess whether the elder's present state allows him safely to engage in
   self- care.


5. The nurse should assess her elders for re- establishing self- care in future
   Planning. The plan of nursing care must be realistic considering the age￾related changes, which affect the elder and the utilization of his remaining
   potentialities. The plan could also be used to document the nursing system
   that will be utilized, (that is wholly, partially compensatory, or supportive
   educative)
   Implementation:
   Nursing actions should be directed towards strengthen self- care capacity,
   minimizing self-care limitations and providing direct services supports
   independence, and teaching another when the elder's demands can't be
   independently fulfilled.
