New Facility: Attachment 2 supplements Attachments 1 and 3 (section 5.2.1). Construction and equipment installation comply with codes, Management Systems, and design. Deviations from approved design require a SHEM-09 Management of Change review prior to startup. Field-installed items, even at installer discretion (e.g., low point drains, gaskets), align with approved design specifications. Pre-operational activities like strainer/blind removal, re-gasket repairs, and leak tests are successfully completed. Field inspection identified unfinished thermal insulation, temporary covers, metal straps, steam trap discharge, scaffolding, and un-welded plugs/nipples. Instrumentation logic philosophy shows no deviations. Protective safety equipment (e.g., safety showers, detectors) availability and functionality confirmed.

New Facility:
• Attachment 2 shall be followed as additional to attachment 1
and 3 mentioned in section 5.2.1.
• Construction and equipment installation is in accordance with
codes, Management Systems and design specifications.
Observed deviations from approved design shall be reviewed
according to Management of Change SHEM-09 prior to startup
• Items not specified in design but located and installed at the
discretion of the installer in the field, such as: low point drains,
gaskets, direction arrows on rupture disks, torque on sight
glasses, location of startup strainers, etc. are in accordance with
approved design specifications.
• Pre-operational activities such as strainer and blind removal, regasket repairs, adjustments and leak tests have been
successfully completed.
• Field inspection items such as unfinished thermal insulation,
temporary covers, metal insulation straps, steam trap
discharge, scaffolding, un-welded plugs and nipples are
identified.
• Checks have been conducted to ensure that there are no
deviations from instrumentation logic philosophy.
• Field safety inspection items such as availability and
functionality of protective safety equipment (safety showers,
detectors, sprinklers, etc.) have been confirmed
