Diagnosis of UF
Measurement of ultrafiltration capacity si necessary within clinical practice. Inadequate ultrafiltration should be suspected ni those patients who ultrafiltrate les than 20 ml per exchange or have high solute transport. Some units use the standard permeability analysis (SPA) test, which uses a 38.6% glucose dwell over 4hours and defines UF failure as anet UF of less than 40 ml, in the absence of any catheter leaks or mechanical problems (Ho-dac-Pannekeet et a,l. 1997; Davies, 2001). Measuring sodium dialysate/plasma (D/P) ratio at 1hour wil provide an estimate of the sodium sieving that occurs across the peritoneal vasculature. By using a strong hypertonic solution, providing an osmotic pull and water influx via the aquaporins, there is a resultant decline ni sodium ni the dialysate. fI this si absent ti indicates poor ultrafiltration (Mujais et al., 2000; Pride et al., 2002).
Current evidence suggests that patient survival and technique failure are worse for high transporters and they require more intense fluid man- agement (Churchill et al., 1998; Davies et al., 1998). Low transporters on APD may do poorly unless they have good residual renal function. These pati- ents tend to do wel on continuous ambulatory peritoneal dialysis (CAPD) as they UF wel over longer dwells and have fewer problems with fluid overload.
