Description of Work: Clearly outline the maintenance tasks to be performed on the electrical motor, including any troubleshooting, inspection, testing, or repair activities.

Location: Specify the exact location of the electrical motor within the industrial facility.

Equipment Involved: Identify the specific electrical equipment involved in the maintenance, including the motor, control panels, disconnect switches, and any other associated components.

Hazard Identification: Conduct a thorough risk assessment to identify potential hazards associated with the maintenance work, such as electrical shock, arc flash, fire, or chemical exposure.

Precautionary Measures: Outline the precautionary measures to be implemented to mitigate identified hazards. This may include:

Lockout/Tagout procedures to isolate electrical power sources.
Use of personal protective equipment (PPE) such as insulated gloves, safety glasses, and arc flash protective clothing.
Verification of de-energization and zero-energy state before beginning work.
Proper grounding and bonding techniques to prevent electrical hazards.
Fire prevention measures, such as ensuring flammable materials are removed from the work area.
Provision of adequate ventilation for confined spaces or areas where hazardous gases may be present.
Emergency procedures in case of accidents or incidents, including evacuation routes and contact information for emergency responders.
