The SOAP note format was developed in the 1960s at the University of
Vermont by Dr. Lawrence Weed as part of the Problem-Oriented Medical
Record (POMR) system. Each patient chart in this system is headed by a
numbered list of patient problems, and when entering documentation,
each professional refers to the number of the problem and then writes a
note using the SOAP format. The SOAP format requires the practitioner to
enter information in the order of the acronym's initials: Subjective,
Objective, Assessment, and Plan. This structured and organized method of
documentation is widely used by healthcare providers to convey patient
progress, assessment, and future actions among care teams. The SOAP
note serves as a cognitive aid and a potential index to retrieve information
for learning from the record, emphasizing clear and well-organized
documentation of findings with a natural progression from the collection
of relevant information to the assessment to the plan on how to proceed
