Chronic Kidney Disease (CKD) is a gradual deterioration in kidney function over months or years. It refers to the inability of the kidneys to effectively filter toxins and fluids from the blood, leading to the accumulation of harmful substances in the body
Causes of Chronic Kidney Failure :



  1. Diabetes: It is the most common cause of chronic kidney failure, as high blood sugar levels damage the blood vessels in the kidneys.

  2. Hypertension : It causes damage to the blood vessels in the kidneys, affecting their ability to filter.

  3. Genetic kidney diseases: such as polycystic kidney disease.

  4. Chronic kidney infections: such as glomerulonephritis, a condition that affects blood filtration.

  5. Poisoning or medication cases: such as the use of some drugs that are toxic to the kidneys, such as painkillers or cancer treatment drugs.

  6. Chronic obstruction of the urinary tract: such as kidney stones or an enlarged prostate.

  7. Recurrent kidney infections: which can gradually damage the kidneys.


Symptoms
Symptoms develop slowly as the disease progresses and may not be apparent in the early stages. However, as the disease progresses, the following symptoms may appear:



  1. General fatigue and weakness.

  2. Swelling (edema) in the feet, ankles or face due to fluid retention.

  3. High blood pressure.

  4. Decrease or change in the amount of urine.

  5. Nausea and vomiting.

  6. Itchy skin.

  7. Loss of appetite.

  8. Chest pain or difficulty breathing (may occur in advanced cases due to fluid accumulation in the lungs).

  9. Changes in urine color (such as dark urine or blood in the urine).

  10. Sleep disturbances


Diagnosis



  1. Blood tests:
    Blood creatinine level: To determine how well the kidneys are filtering.
    Glomerular filtration rate (GFR): To assess how well the kidneys are functioning.
    Urea level: To check the level of toxins in the blood.

  2. Urine tests:
    Urine analysis to detect protein or blood.
    Urine albumin test (a sign of kidney damage).

  3. Imaging:
    Ultrasound or CT scan of the kidneys to identify any abnormalities or blockages.

  4. Renal biopsy: In some cases to determine the exact cause of kidney deterioration.


Treatment
Treatment of chronic renal failure depends on the underlying cause and how advanced the disease is. The main goals are to slow the progression of the disease, control symptoms, and maintain kidney function.




  1. Treating the underlying cause:
    Control diabetes: Control blood sugar levels using appropriate medications.
    Control high blood pressure: Using medications such as angiotensin-converting enzyme (ACE) inhibitors or angiotensin receptor blockers.
    Treatment of other kidney diseases: Such as immunosuppressive medications in the case of immune-mediated nephritis or treatment of infection.




  2. Medications:
    Diuretics: Help get rid of excess fluid.
    Medications to lower potassium and phosphorus levels in the blood.
    Anemia treatment: such as medications containing erythropoietin.
    Medications to control cholesterol levels.




  3. Diet modification:
    Reducing protein intake: to reduce the workload on the kidneys.
    Reducing sodium (salt) intake to reduce fluid retention.
    Reducing potassium and phosphorus in the diet.
    Monitoring fluid intake: The doctor may recommend limiting the amount of fluids that can be consumed.




  4. Dialysis:
    In advanced stages of the disease, the kidneys may not be able to do their job. Dialysis is used to remove toxins and excess fluid from the blood.
    Hemodialysis: is done by a machine to purify the blood.
    Peritoneal dialysis: is done by a wash inside the abdominal cavity to filter the blood.




  5. Kidney transplant:
    In very advanced cases where the kidneys stop working completely, a kidney transplant from a suitable donor may be considered.

Managing chronic diseases: such as diabetes and high blood pressure effectively.
Eating a healthy diet: includes reducing salt and protein intake.
Exercise regularly: to improve heart health and control weight.
Stop smoking: Smoking increases the risk of kidney disease.
Regular medical follow-up: to monitor kidney function, especially for people at risk.
