Mr. Faihan Al-harbi is a 52 years old male Saudi who lives in Al-Madinah and he is married with the children and who works as a school teacher Mr. Faihan was in his usual health state until his presentation in King Fahad Hospital ER on 24th of February after midnight, he complained from chest pain before 4-hour, the Chest pain attack last longer than 30min , the pain was developed suddenly in his retrosternal area and the patient describe the character of the pain as Pressure like sensation ( a heavy weight sitting on my chest ) that radiates to his left arm particularly in the inner side of the arm and his lower jaw , the pain is progressive in course worsened with exertion and emotional stress and partially relieved with resting and using sublingual nitroglycerin as they give him in the ER , the patient described the severity of the pain 9 out of 10 that interferes with his daily activities, As for the associated cardiovascular symptoms (7) , The pain was associated generalized fatigue and shortness of breath (Dyspnea) that was fluctuating in course come with exertion and associated with cough also the patient Reports that he had intermittent palpitations during the past week. The patient denied syncope, lower limb edema , cough and wheezing. Hypertension: Diagnosed 10 years ago, on medication.
