Four children-two sons and two daughters-are raised by client M.A., who is widowed. Therefore, the client depends on the pension money from her late spouse and her oldest son's help.

Referral Source and Presenting Problem: Subject M.A.was approach in order to fulfill the partial requirement of course assignment. Assessment was conducted at her place. and the 15 Words Test assesses her capacity to verbally encode, mix, store, and retrieve information at various stages of short-term memory.

M.A. was also selected because, according to the client, she occasionally forgets where she puts things or, to put it another way, she forgets when someone says an important thing to her.

Interview Information: According to the client, she occasionally forgets where she put things or, to put it more succinctly, she forgets when someone says something important to her. Since her husband passed away four years ago, she has been solely responsible for her children. As she does not work, she enjoys making her home a cozy place for her family and herself. She continued by saying that she adores baking in particular.

learning approach, with the participant listening to a list of 15 nouns and asked to remember as many words as possible. Bean, J. (2011). The test is conducted five times, with the examiner reading aloud at a rate of one per second. After the first trial, the participant is asked to recall as many words as possible, including those said the first time. Wilkinson, A. (2022). The RAVLT scores are derived from raw RAVLT scores, including RAVLT Immediate, Learning, and Forgetting. The test is designed to assess memory impairment and monitor memory function changes. Bean, J. Behavior During Testing: Subject M.A. showed signs of calm and comfort during the test, there

was no pressure during the answers, and she was relaxed when she remembered the words, the

environment during the test was appropriate and calm, and we were not interrupted during the

test.

Evaluation and Interpretation: Her total score was 61. Her scores on the trails were 9,11,12,14,15 respectively.

The total of her score was 61 out of 75, which indicates that it is above average. In the first trial, she scored 9 words out of 15 words. In each trial, the number of words increased, which shows learning and improvement during the memory of words. It is possible that the environment during the test was one of the auxiliary factors for it to be quiet and comfortable during the test to remember the words. It was noted that in each round it is associated between (plane, moon) and (book, finger). This indicates its good ability at connecting and linking to words.
